Terms and conditions for STEAM Camp 1-Week pass

Parental Waiver and Release of Liability

I , the undersigned parent / legal guardian of the enrolled student listed on this pass hereby represent that I am in fact acting in such capacity and agree to save and hold harmless and indemnify MIRACLE MATH COACHING and/or its designated representatives from any liability and all damages in the event of injury resulting from my child/ward’s participation in Miracle Math Coaching Workshops and Academic Coaching Services.

I hereby agree to indemnify, save and hold harmless MIRACLE MATH COACHING, and/or its designated representatives, from any loss, liability, damage or cost that may incur due to the presence of the undersigned and/or the undersigned’s child or ward in or upon Miracle Math Coaching Workshop and Academic Coaching Services, facility or grounds, or in any way during math sessions, observing or working for or for any purpose participating in the program or activities.

THE UNDERSIGNED HAS READ AND VOLUNTARY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT and further agrees that no oral representation, statements or inducement apart from the foregoing written agreement have been made.

This waiver release and indemnification agreement specifically embraces each and every activity sanctioned or promoted by said release during the entire duration of the program and applies to each and every session, or activity herein mentioned and has the same effect as executed after each and every activity or event in which the undersigned participates so that the parties herein intended to be released and indemnified shall be fully and effectively released and indemnified as to each and every activity herein described.

Parent Release for Media Recording ( Our audiences are all family-related)

I , the undersigned, do hereby grant to MIRACLE MATH COACHING to use any media collected during the activity I have signed my student(s) up.

Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Miracle Math Coaching website.

I understand and accept that my child’s images may possibly be used for educational social media post or materials. This could be either within Miracle Math Coaching or STEAM Camps in the larger community. One example of this could be videos in parent education classes.
